Job summary

OneWell Health Care seeks a Per Diem Physical Therapist to deliver personalized therapy to individuals with IDD in home and community settings in Pennsylvania. You will evaluate needs, develop goals, and collaborate with caregivers to improve mobility, balance, safety, and independence.

The role offers flexible scheduling, field-based work, and a supportive, patient-centered team focused on high-quality care and lasting relationships with clients and families.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required per organizational policy).
• Strong clinical, documentation, organizational, and communication skills.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function, and independence.
• Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
