Job summary

OneWell Health Care is seeking a dedicated Per Diem Physical Therapist in Vintondale, PA to deliver personalized therapy in home and community settings. The role emphasizes evaluating mobility, strength, balance, gait, and safety, with goal-based treatment plans and collaboration with caregivers.

You will provide evidence-based PT, help improve independence, and use mobility aids and adaptive equipment as needed. Strong documentation and communication are essential for success.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification (preferred or required by policy).

Responsibilities

• Evaluate mobility, strength, balance, gait, posture and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function and independence.
• Recommend mobility aids and adaptive equipment; design fall-prevention strategies.
• Educate individuals, families and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
