Job summary

OneWell Health Care is seeking a dedicated Physical Therapist (PT) for a per-diem role in Pottstown, PA. The PT will provide personalized therapy to individuals with IDD in home and community-based settings, evaluating needs, creating goal-based plans, and collaborating with caregivers to improve mobility and independence.

Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current license as Physical Therapist in the state of practice.
• Current CPR certification (preferred or required per policy).

Responsibilities

• Assess mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based PT in home and community settings.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Complete accurate and timely clinical documentation.
• Travel to home and community-based service locations as needed.
