Job summary
OneWell Health Care is seeking a compassionate Per Diem Physical Therapist to provide personalized therapy for individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania.
You will evaluate each individual's physical needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, balance, safety, and overall independence.
Qualifications
- Must hold a Physical Therapy license in Pennsylvania (PT)
- Experience delivering home health PT services to IDD populations preferred
- Current CPR certification and clean driving record required or as per policy
Responsibilities
- Evaluate mobility, strength, balance, gait, posture, and safety needs.
- Develop and update individualized treatment plans.
- Provide evidence-based PT in home and community settings.
- Help individuals improve mobility and independence.
- Recommend mobility aids and therapeutic exercises.
- Educate patients, families, and caregivers on safe mobility.
- Collaborate with caregivers and interdisciplinary team.
- Complete accurate clinical documentation.
- Attend required meetings and trainings.
