Job summary

BrightStar Care is seeking a Physical Therapist to join our Therapies Team in the Duluth and surrounding communities. You will provide compassionate home health physical therapy, delivering personalized care and supporting patient outcomes in a collaborative environment.

Ideal candidates have a PT degree, MN license, and 4+ years of experience; you will assess, plan, and implement therapies, document progress, and work with physicians and family members.

Qualifications

• Experience applying physical therapy modalities and clinical decision making.
• Ability to work in patients’ homes and collaborate with care teams.
• Excellent communication with patients, families, and physicians.

Responsibilities

• Assess, plan, implement, and evaluate physical therapy modalities for patients.
• Follow department policies, procedures, and competency guidelines.
• Improve or minimize residual physical disabilities to restore function.
• Return patients to optimum and productive level within their capabilities.
• Collaborate with home care team in patient care planning.
• Take initial history and perform initial evaluation.
• Consult with physicians on patient progress.
• Participate in quality assessment and improvement activities.
• Prepare clinical and progress notes.
• Assist in developing and revising the plan of care.
• Consult with family and agency staff.
• Regular bending, lifting and standing as required.

Key Responsibilities:

• Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
• All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive level within patient’s capabilities.
• Participates with all other home care personnel in patient care planning.
• Takes initial history and makes initial evaluation.
• Consults with physicians regarding patient’s progress
• Participates in quality assessment and performance improvement activities as assigned.
• Prepares clinical and progress notes.
• Helps develop the plan of care and revise, as necessary.
• Consults with family and agency personnel.
• Ability to do extensive bending, lifting and standing on a regular basis.

Requirements:

• A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
• An active license to practice Physical Therapy in MN
• A minimum of four (4) years’ experience as a Physical Therapist, home care experience a plus
• Must have criminal background check
• Must have current CPR certification
• Must have active driver's license and clear driving record
• Computer skills.
• Ability to exercise initiative and independent judgment
