Job summary

BrightStar Care Northwest Compassionate Care, LLC is seeking a Per Diem Physical Therapist (DPT) to join a team delivering client-centered care in the St. Cloud area. You will travel to clients’ homes, evaluate needs, and implement plans of care with a focus on geriatric clients.

This flexible, per-diem role offers weekly pay and the chance to contribute to a growing home-care network. You’ll work with a schedule created from availability, earning $70 per visit and $100 per evaluation, with 5–15

Qualifications

• Minimum 1 year of PT experience, geriatric population preferred.
• Active Minnesota PT license.
• Current CPR certification.
• Valid driver's license and reliable vehicle for work.

Responsibilities

• Assess and evaluate clients and develop plans of care.
• Perform visits per established treatment plan.
• Coordinate with care teams to support client outcomes.

A day in the life of our Physical Therapist:

As an Physical Therapist (PT), you will assess and evaluate, develop the plan of care and treatment. Participate in the development of the client's total plan of treatment and perform visits based on treatment plan

BrightStar PT Requirements:

• 1 year of experience as Physical Therapist, with Geriatric population preferred.
• Valid driver's license, Auto Insurance and reliable vehicle to use for work
• Current CPR Certification
• Current PT license for Minnesota
