Mount Sinai Rehabilitation Hospital is seeking a Primary Therapist / Social Worker to deliver comprehensive behavioral health care, coordinating services with an interdisciplinary team to meet the needs of patients and their families. The role involves intake and discharge documentation, developing treatment plans, referrals, and collaboration with outside agencies to support care continuity.

A master's in behavioral health and CT licensure are preferred.

Qualifications

• Master’s degree in behavioral health from an accredited college or university.
• CT licensure in appropriate designation preferred (LM/LCSW, Psychologist, LMFT, or LPC).
• Previous behavioral health experience or internship preferred.

Responsabilités

• Completes intake, assessment, treatment planning and discharge documentation.
• Collaborates with clinical program staff to explore appropriate treatment options.
• Establishes relationships with patients and families to set reasonable expectations for care.
• Completes aftercare referrals and coordinates with outside agencies as needed.
• Demonstrates problem solving, conflict resolution, and positive communication through teamwork.
