Job summary

Mount Sinai Rehabilitation Hospital, part of Trinity Health, seeks a Primary Therapist / Social Worker to coordinate behavioral health care services and drive an interdisciplinary, patient-centered approach. The role emphasizes case management, collaboration with the care team, and delivering compassionate, comprehensive care to patients and families.

Responsibilities include intake and discharge documentation, treatment planning, and coordination with external agencies.

Qualifications

• Master’s degree in behavioral health from an accredited college or university.
• CT licensure in appropriate professional designation preferred (e.g. LM, LCSW, Psychologist, MFT, or Professional Counselor).
• Previous behavioral health experience or internship work preferred.

Responsibilities

• Completes intake, assessment, treatment planning and discharge documentation.
• Collaborates with clinical program staff to explore treatment options.
• Establishes relationships with patients and families to set expectations for care.
• Completes all aftercare referrals and collaborates with outside agencies as needed.
• Demonstrates problem solving, conflict resolution, and teamwork in communication.
