Job summary

Human Touch Home Health is seeking a compassionate Home Health Physical Therapist (PT) to join our team serving patients across Northern Virginia, including Falls Church and surrounding communities. This PRN position offers professional independence, flexible scheduling, and the chance to impact lives daily.

You’ll provide one-on-one care, develop individualized treatment plans, and help patients regain mobility and confidence in their homes with strong clinical leadership support.

Qualifications

• Active Virginia Physical Therapist License required.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment, communication, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Complete physical therapy evaluations, reassessments and discharge visits.
• Develop individualized plans of care based on patient goals and clinical needs.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on therapeutic exercises, mobility training, balance, fall prevention, and home safety.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.
