Job summary

Human Touch Home Health is seeking a compassionate Home Health Physical Therapist (PT) to join our growing team serving patients throughout Washington, DC. This PRN opportunity offers flexibility, professional independence, and the ability to make a meaningful impact with each visit.

As a Home Health PT, you will perform evaluations, develop plans of care, educate patients and caregivers, and collaborate with physicians.

Qualifications

• Active Washington, DC Physical Therapist license required.
• Valid drivers license and reliable transportation.
• Strong clinical assessment and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Perform physical therapy evaluations, reassessments, and discharge visits.
• Develop individualized plans of care based on patient goals and clinical needs.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, balance, and home safety.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.

• Home Health Experience Preferred but Not Required

• Experience with WellSky Home Health is a plus
