Job summary

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Washington, DC. As a Home Health PT, you'll provide individualized care in patients' homes, develop personalized treatment plans, educate families, and collaborate with physicians and the care team.

Qualifications

• Active Washington, DC Physical Therapist License required.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Home health experience preferred but not required.
• OASIS experience is a plus.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Complete evaluations, reassessments, and discharge visits.
• Develop individualized plans of care based on patient goals and clinical needs.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with physicians and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.
