Job summary

Center of Illinois Home Healthcare, Inc. is seeking a Per Diem Physical Therapist to deliver skilled services in patients' homes across Illinois, including Arlington Heights and McHenry County. The role requires an IL license and CPR, with up to 25% travel.

Flexible per diem schedule and competitive per-visit compensation are offered. Responsibilities include evaluating patients, developing treatment plans, and educating families. OASIS experience and EMR use (Axxess) are preferred.

Qualifications

• Current Illinois Physical Therapist license is required.
• Home health experience preferred.
• OASIS experience is preferred.
• CPR certification is required.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care.
• Assess patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
• Complete timely documentation per Medicare and CMS guidelines.
• Collaborate with interdisciplinary team and participate in care conferences.
• Maintain compliance with agency policies and state regulations.

Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care.
• Assess patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on therapeutic exercises, mobility, fall prevention, and home safety.
• Complete timely and accurate documentation in accordance with Medicare, CMS, and agency guidelines.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary team.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies, state regulations, and accreditation standards.

Qualifications:

• Current Illinois Physical Therapist license required.
• Minimum of one year of home health experience preferred.
• Experience with Medicare home health regulations and documentation preferred.
• OASIS experience preferred.
• Experience with Axxess EMR is a plus.
• Excellent communication, clinical assessment, and organizational skills.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
