Job summary

Center of Illinois Home Healthcare, Inc. is seeking an experienced Per Diem Registered Nurse (RN) to provide skilled home health nursing services in patients' homes across Illinois.

The role requires an active Illinois RN license, at least one year in home health, and proficiency with OASIS documentation. You will perform SOC/ROC/Recert/Routine/Discharge visits, educate patients and caregivers, and coordinate with physicians and the care team, while maintaining compliant documentation and

Qualifications

• Active Illinois RN license.
• Minimum of 1 year of home health nursing experience.
• Experience with OASIS documentation.
• Knowledge of Medicare home health regulations.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing visits in patients’ homes.
• Complete SOC, ROC, Recert, Routine and Discharge visits.
• Accurately complete OASIS assessments and documentation.
• Educate patients and caregivers on disease management, medications and safety.
• Coordinate care with physicians and the interdisciplinary team.
• Monitor patient progress and report changes promptly.
• Ensure compliance with Medicare, CMS, ACHC and agency policies.
• Maintain accurate electronic documentation and participate in case conferences.
