Job summary

Center of Illinois Home Healthcare, Inc. is seeking a per diem registered nurse to perform skilled nursing visits in patients' homes throughout Illinois. You will complete SOC, ROC, recertifications, and discharge visits, and document in a timely manner.

The role requires an active Illinois RN license, 1 year of home health experience, CPR, and strong assessment and communication skills. Flexible scheduling and patient-home visits available across Illinois.

Qualifications

• Active Illinois Registered Nurse (RN) license.
• Minimum of one (1) year of home health nursing experience.
• Knowledge of OASIS documentation.
• Knowledge of Medicare home health regulations.
• Current CPR certification.
• Valid driver''s license, reliable transportation, and current auto insurance.
• Strong assessment, communication, and time-management skills.

Responsibilities

• Provide comprehensive skilled nursing visits in patients' homes.
• Perform patient assessments and develop individualized nursing care plans.
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, and Discharge visits.
• Accurately complete OASIS assessments and clinical documentation within required timeframes.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate patient care with physicians and the interdisciplinary care team.
• Monitor patient progress and promptly report changes in condition.
• Ensure compliance with Medicare, CMS, ACHC, and agency policies and procedures.
• Maintain accurate, timely, and complete electronic documentation.
• Participate in case conferences and other meetings as needed

Per Diem / PRN Flexible scheduling based on patient needs Weekend and holiday availability may be requested

Work Location

Patient homes throughout the agency's service area in Illinois. More commonly in Lake, Cook, McHenry, Kane, and DuPage counties.
