Center of Illinois Home Healthcare, Inc. is hiring a Full-time Registered Nurse (RN) for home health in Arlington Heights, IL. You will perform skilled nursing visits, assess patients, and develop care plans in collaboration with physicians and the interdisciplinary team.

Ideal candidates have an Illinois RN license, 2+ years of home health experience, and OASIS knowledge; CPR is required. The role involves independent work, travel to patient homes, and coordination with therapists and aides.

Qualifications

• Active Illinois RN license required.
• Minimum 2 years home health nursing experience preferred.
• OASIS experience preferred.
• Strong knowledge of Medicare home health regulations.
• CPR certification required.
• Experience with Axxess EMR preferred.

Responsibilities

• Provide skilled nursing visits and patient assessments in the home.
• Develop and update individualized care plans with physicians and team.
• Complete OASIS assessments and clinical documentation accurately.
• Educate patients and families on disease management and home safety.
• Monitor progress and communicate changes to physicians and team.
• Coordinate with therapists and home health aides to ensure integrated care.
• Ensure CMS/ACHC/Medicare compliance and follow agency policies.
• Maintain EMR documentation and participate in QA initiatives.

Full‑time Registered Nurse (RN) – Home Health – Arlington Heights, IL

Responsibilities

• Provide skilled nursing visits and comprehensive patient assessments in the home.
• Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Complete accurate and timely OASIS assessments and clinical documentation.
• Educate patients, families, and caregivers on disease management, medications, treatments, and home safety.
• Monitor patient progress and communicate changes in condition to physicians and other healthcare providers.
• Coordinate care with physical therapists, occupational therapists, speech therapists, home health aides, and other members of the care team.
• Ensure compliance with CMS, ACHC, Medicare, and agency policies and procedures.
• Participate in case conferences and quality improvement initiatives.
• Maintain accurate documentation using the agency's electronic medical record (EMR) system.

Qualifications

• Active Illinois Registered Nurse (RN) license.
• Minimum of 2 years of home health nursing experience preferred.
• OASIS experience preferred.
• Strong knowledge of Medicare home health regulations.
• Experience with Axxess EMR preferred.
• Excellent assessment, organizational, and communication skills.
• Ability to work independently while managing multiple patient visits.
• Current CPR certification required.
