Requirements include IL PTA license, CPR, driver’s license, and 1 year home health experience preferred. Knowledge of Medicare regulations and Axxess EMR experience are a plus.

Qualifications

• Current Illinois PTA license required.
• Home health experience preferred (1 year).
• Knowledge of Medicare home health regulations and documentation preferred.
• CPR certification required.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide skilled physical therapy treatments under supervising Physical Therapist.
• Follow plan of care and treatment goals established by supervisor.
• Observe, document, and report patient responses; communicate changes to supervisor.
• Educate patients and caregivers on exercises, mobility, fall prevention, and home safety.
• Complete accurate clinical documentation per Medicare CMS and agency rules.
• Collaborate with nurses, therapists, physicians, and care team members.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies and state regulations.

Center of Illinois Home Healthcare, Inc. is seeking a compassionate and dependable Per Diem Physical Therapist Assistant (PTA) to join our Medicare-certified, ACHC-accredited home health agency. This position offers a flexible schedule while providing high-quality physical therapy services under the supervision of a licensed Physical Therapist in patients' homes.

As a valued member of our interdisciplinary team, you will help patients improve mobility, restore function, reduce pain, and maximize independence in the comfort of their homes.

Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Follow the established plan of care and treatment goals developed by the supervising Physical Therapist.
• Observe, document, and report patient responses to treatment and communicate changes in condition to the supervising Physical Therapist.
• Educate patients and caregivers on prescribed exercises, mobility techniques, fall prevention, and home safety.
• Complete accurate and timely clinical documentation in accordance with Medicare, CMS, and agency guidelines.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary care team.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies, state regulations, and accreditation standards.

Qualifications

• Current Illinois Physical Therapist Assistant (PTA) license required.
• Minimum of one year of home health experience preferred.
• Knowledge of Medicare home health regulations and documentation preferred.
• Experience with Axxess EMR is a plus.
• Strong clinical, communication, and organizational skills.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
