Job summary

Center of Illinois Home Healthcare, Inc. is seeking an experienced Per Diem Occupational Therapist to join our Medicare-certified, ACHC-accredited home health agency serving Arlington Heights, IL.

The OT will evaluate patients in their homes, develop personalized treatment plans, educate caregivers, ensure compliant documentation, and collaborate with an interdisciplinary team to improve safety and independence.

Qualifications

• Active Illinois OT license is required.
• Graduate of an accredited OT program (OT degree).
• Minimum of 1 year clinical experience; home health experience preferred.
• Knowledge of Medicare/home health regulations and documentation requirements.
• CPR certification is required.

Responsibilities

• Evaluate patients' functional abilities and develop individualized OT treatment plans.
• Provide skilled OT services per physician orders and Plan of Care.
• Assess ADLs and promote independence.
• Educate patients and caregivers on home safety and fall prevention.
• Collaborate with physicians, nurses, and other therapies to coordinate care.
• Monitor progress and modify treatment plans as needed.
• Complete documentation per Medicare CMS guidelines.
• Participate in case conferences and team meetings as needed.
• Maintain professional standards and outcomes.

Per Diem Occupational Therapist (OT), Home Health

Responsibilities

• Evaluate patients' functional abilities and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in accordance with the physician's orders and the established Plan of Care.
• Assess patients' ability to perform activities of daily living (ADLs) and recommend strategies to improve independence.
• Educate patients and caregivers on adaptive techniques, home safety, fall prevention, and the use of adaptive equipment.
• Collaborate with physicians, nurses, physical therapists, speech therapists, home health aides, and other members of the interdisciplinary team to coordinate patient care.
• Monitor patient progress and modify treatment plans as appropriate.
• Complete accurate, timely, and compliant documentation in accordance with Medicare, CMS, and agency guidelines.
• Participate in case conferences and interdisciplinary team meetings as needed.
• Maintain professional standards of practice while ensuring quality patient outcomes.

Qualifications

• Active Illinois Occupational Therapist (OT) License required.
• Graduate of an accredited Occupational Therapy program.
• Minimum of one year of clinical experience; home health experience preferred.
• Knowledge of Medicare home health regulations and documentation requirements.
• OASIS experience preferred.
• Experience with Axxess EMR preferred.
• Excellent communication, organization, and time-management skills.
• Ability to work independently and manage a flexible patient schedule.
• Current CPR certification required.
• Valid driver's license, reliable transportation, and current auto insurance required.
