Job summary

Rivertown Health and Rehab is hiring a Physical Therapist PT on a per diem/PRN basis to join our Therapy Team. You will plan, organize, develop, and direct Physical Therapy services in accordance with applicable standards and as directed by Administration to ensure high-quality resident care.

Responsibilities include evaluating residents, developing treatment plans with short/long-term goals, documenting per CMS expectations in NetHealth EMR, and collaborating with an interdisciplinary team.

Qualifications

• Must hold an active Physical Therapy license in AL.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services per standards and department procedures.
• Document and bill therapy services in NetHealth EMR.
• Evaluate residents to plan and implement treatment; communicate findings to team and families.
