Job summary

Bridgeway Health and Rehab is seeking a Physical Therapist (PT) to join our Therapy Team. You will plan, organize, develop, and direct Physical Therapy services in accordance with state and federal standards to ensure high-quality resident care.

The position is Per Diem/PRN with duties including evaluation, treatment planning, documentation in NetHealth EMR, and collaboration with an interdisciplinary team.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in AL.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary teams.
