Job summary

Hireology is seeking a Per Diem Physical Therapist to join our in-house rehab team serving residents with person-centered care. The PT will assess, treat, and document therapy services, building individualized rehabilitation plans and collaborating with the interdisciplinary team.

Texas license is required, and SNF/long-term care experience is preferred. This PRN role offers flexible scheduling, on-site work, and non-contracted rehab opportunities within our facility.

Qualifications

• Active/valid Texas license in Physical Therapy is required.
• Experience in SNF/long-term care is preferred.
• Strong documentation and compliance with regulatory requirements.

Responsibilities

• Consult with patients to assess physical condition and develop plans.
• Administer PT treatments and monitor patient progress.
• Document care thoroughly and timely per plan of care.
• Collaborate with interdisciplinary team to ensure quality care.
