Caring Hand Home Health Solutions, Inc. is seeking a Physical Therapist to deliver skilled care in home health settings in California. Per diem arrangement with a rate of $160–$180 per visit.

You will evaluate functional status, mobility, and ROM; develop treatment plans; educate patients and families; and collaborate with physicians and the care team. Eligible candidates have an CA PT license, 2+ years of PT experience, CPR, and a valid driver’s license.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• CPR certification (AHA).
• Valid driver’s license with reliable, insured vehicle.

Responsabilités

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
