Job summary

Caring Hand Home Health Solutions, Inc is seeking a Physical Therapist to deliver skilled services in patients' homes across California. The role focuses on mobility, independence, and safety, aligning with physician plans and home health regulations.

The position is per diem, with a rate of $160 - $180 per visit. Candidates should have CA PT license, a Bachelor's or Master's in PT, CPR/AHA, driver's license, and at least 2 years of PT experience, preferably in home health.

Qualifications

• PT degree from an accredited program and CA license.
• Minimum 2 years PT experience; home health a plus.
• Strong communication with patients and families.
• Current CPR certification (AHA) and valid CA driver's license.

Responsibilities

• Evaluate patients’ functional status, mobility, strength, gait, and ROM to plan care.
• Deliver PT services per physician plan and CA PT Act.
• Educate patients and families on exercises, equipment, and goals.
• Collaborate with physicians and care team; report changes.
• Supervise PTAs and oversee Home Health Aide when needed.
• Manage documentation including progress notes and discharge planning.
• Identify needs for additional home health services and referrals.
• Perform other duties for patient services.
