Caringhand Home Health Solutions, Inc in California is seeking a Physical Therapist to deliver skilled care in patients' homes. This field-based, per diem role requires travel to patients and adherence to California's Physical Therapy Practice Act.

You will evaluate functional status, design treatment plans, educate families, and coordinate with physicians. A CA license, CPR certification, and a driver’s license with reliable vehicle are required; 2+ years of PT experience, preferably in home

Formación

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsabilidades

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
