Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem Physical Therapist to provide skilled services in patients' homes in California.

You will assess mobility, develop individualized treatment plans, and implement therapy aimed at regaining independence and confidence. Requirements include a PT degree, active California license, a minimum of 2 years’ PT experience (home health a plus), CPR certification, and a valid driver’s license.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Active California Physical Therapy license.
• CPR certification (AHA).

Responsibilities

• Evaluate patients’ functional status, mobility, strength and ROM to tailor plans.
• Deliver PT services per physician’s plan and California law.
• Educate patients and families on exercises, equipment and goals.
• Collaborate with care team and report changes in condition.
• Supervise Physical Therapy Assistants and oversee Home Health Aide when needed.
• Manage documentation including progress notes and discharge planning.
• Identify needs for additional home health services and referrals.
• Perform other duties as required for patient services.
