Job summary

CHRISTUS Good Shepherd HomeCare, part of the LHC Group and Optum family, seeks a Per Diem Physical Therapist Assistant to deliver compassionate home-based therapy, following the established plan of care and aiming to improve patient well-being.

You will provide exercise instruction, patient education, and coordinate with the supervising PT, nurses and physicians across diverse home settings, ensuring safe, effective therapy and timely communication of patient progress.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Valid driver's license and access to reliable transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow established plan of care under supervision of PT.
• Instruct patients in active and passive exercises and gait training.
• Observe and report patient responses and coordinate with healthcare team.
• Educate patients and families on therapy and home exercises.
• Perform therapy activities in diverse home environments.
