Job summary

Mederi Caretenders, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to provide treatment and patient education under the plan of care in home health settings.

You will perform exercise instruction, monitor patient progress, and coordinate with supervising PT to optimize outcomes while adhering to state laws and facility policies. This role emphasizes flexibility and patient-centered care across diverse home environments.

Qualifications

• Current, licensed, registered, or certified to assist in physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing home PT; perform only procedures qualified and trained; suggest changes to plan of care to supervising PT.
• Instructs and aids patients in active and passive exercise, gait, ADL, transfer, and mobility training.
• Observe, record, and report patient responses to treatment to PT, nurse supervisor, or physician; coordinate care with the team.
• Instructs patient, family, caregiver, and other health care team members in therapy within PT assistant scope.
• Function effectively in any home situation regardless of patient background.
