Job summary

Omega Home Health Care Services Inc. seeks a compassionate Occupational Therapist to join our home health team on a per diem basis, delivering rehabilitative services in patients' homes under physician orders.

Candidates must hold a California OT license and have experience in home health settings. A valid driver's license and reliable transportation are required to travel within a designated region, with an emphasis on patient safety and quality of care.

Qualifications

• Licensed Occupational Therapist in California.
• Graduate of an accredited Occupational Therapy program.
• Minimum of 1 year experience in home health or similar setting.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.
• Experience with EMR systems is a plus.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients’ homes per physician orders and company protocols.
• Instruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skills.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with multidisciplinary team including nurses, physical therapists, and social workers.
• Maintain accurate and timely documentation in compliance with California state and federal regulations.
• Ensure patient safety and adherence to infection control procedures in home environments.
