Job summary

Omega Home Health Care Services, Inc. seeks a compassionate Occupational Therapist for per diem home health visits in California.

The role involves evaluating patients, creating and adjusting treatment plans, and educating patients and caregivers on adaptive strategies to improve daily living activities. Qualified candidates hold a California OT license, have completed an accredited OT program, and preferably possess 1 year of home health experience.

Qualifications

• California OT license required.
• Graduate of an accredited OT program.
• Minimum 1 year home health experience preferred.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver occupational therapy services in patients' homes per physician orders and protocols.
• Instruct patients and caregivers in adaptive equipment and techniques.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nurses, PTs, social workers and other team members.
• Maintain accurate documentation in line with state and federal regulations.
• Ensure patient safety and infection control in home environments.
