Job summary

Hanford Post Acute is seeking a Per Diem Physical Therapist to join our rehabilitation team in Hanford, CA. The role offers part-time, per diem hours and a compensation range of $65-$75 per hour based on experience.

The PT will evaluate patients, develop and implement treatment plans, and supervise assistants while coordinating with the interdisciplinary team. California licensure and CPR/CEU requirements apply.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Current California PT license in good standing.
• CPR certification and CEUs as required by the state.
• Experience with Casamba and PointClickCare is preferred.
• Experience in SNF or post-acute care preferred.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral; document any delay.
• Develop individualized treatment plans and obtain physician approval.
• Deliver care as outlined in the treatment plan.
• Supervise PT assistants and trainees as applicable.
• Communicate with interdisciplinary team regarding patient progress and care planning.
• Participate in patient care conferences and utilization reviews.
• Maintain accurate documentation of evaluations, notes, progress, and recertifications as required.
• Instruct caregivers on maintenance programs and post-discharge care.
• Assist in ordering durable medical equipment to enhance mobility.
• Report and monitor maintenance needs of therapy equipment.
