Job summary

Elara Caring is seeking an Occupational Therapist to join our home health team in Dallas, TX. You will evaluate patients, develop a plan of care, and coordinate with physicians to ensure quality, discharge planning and follow-up assessments.

You will guide patients and families in activities of daily living, supervise clinical staff, and travel within the coverage area. A strong OT background and licensed unrestricted status are required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluates the OT patient to determine rehabilitation needs and develops a plan of care.
• Participates in implementation and development of the plan of care and discharge planning.
• Communicates significant findings to supervisor, physician, and team members.
• Initiates safety plans using patient, family, and community resources.
• Writes physician orders to cover additional visits and changes to the plan of care.
• Incorporates patient care goals into therapy care and documents in notes.
• Provides OT services per accepted standards of practice and physician approval.
• Instructs patients and families in activities of daily living using adaptive equipment.
• Assists in securing necessary equipment and fabrication of splints if needed.
• Instructs home therapy program and provides modifications as necessary.
• Supervises/ directs COTA, HHA, and clinical staff to ensure quality care.
• Physical demands include lifting and high-stress environment; travel 50% may be required.
