Job summary

Elara Caring is seeking an Occupational Therapist to deliver high-quality care in patients' homes across the branch. You will evaluate, plan, and implement therapy with a focus on ADLs and safe home environments.

Responsibilities include coordinating with supervisors and physicians, training patients and families, and guiding COTA and aides while maintaining compliance and documentation. A valid OT license and travel up to 50% are required.

Qualifications

• Graduate of an accredited OT program.
• Current, unrestricted OT license in the state of work.
• Minimum one year of clinical care experience.
• Willing to travel 50% within branch/office coverage area.
• Valid driver’s license and dependable vehicle.

Responsibilities

• Evaluate patients to determine rehabilitation needs and develop a plan of care.
• Implement and update the plan of care and re-evaluate as needed.
• Communicate findings to supervisor, physician and care team.
• Instruct ADLs and adapt equipment for home use.
• Supervise COTA and other staff to ensure quality care.
• Maintain therapy documentation and ensure compliance with standards.
