Job summary

Elara Caring is seeking an Occupational Therapist to join our home health team. You will evaluate patients, develop individualized treatment plans, and work to improve independence and daily living skills in a home setting.

You will educate patients and caregivers on home therapy programs, monitor progress, and coordinate adaptive equipment. This role includes supervising COTAs and aides, with travel between patient homes as needed.

Qualifications

• Unrestricted OT license valid for the state of work.
• Graduate of an accredited Occupational Therapy program.
• Minimum one year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Evaluate patients to identify OT needs and develop individualized treatment plans.
• Provide OT to improve independence, daily living skills, fine motor coordination, and overall function.
• Educate patients and caregivers on home therapy programs, adaptive techniques, and equipment.
• Monitor patient progress and adjust treatment plans based on changing needs and physician orders.
• Communicate patient progress, changes in condition, and safety concerns to physicians and the care team.
• Coordinate adaptive equipment and specialized devices, including custom splints when needed.
• Provide guidance and supervision to COTAs, Home Health Aides, and other clinical staff involved in patient care.
