Job summary

Hope Creek Nursing and Rehabilitation Center in East Moline, IL seeks an Occupational Therapist to join our Per Diem team, delivering compassionate, high-quality care to our residents in a supportive setting.

You will evaluate patients, develop and implement individualized OT plans, lead therapy sessions, educate families on home programs, and collaborate with an interdisciplinary team to restore independence and improve daily functioning.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current state licensure and certification as an Occupational Therapist (OT).
• Ability to pass a background check and comply with our facility's policies and procedures.

Responsibilities

• Evaluate patients' physical and cognitive abilities and develop treatment plans to address their specific needs.
• Implement and modify occupational therapy programs to promote patient progress and achievement of goals.
• Provide individual and group therapy sessions, including exercises, activities, and education.
• Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans.
• Educate patients and their families on home programming and maintenance to support continued progress.
• Document patient progress and treatment plans in a timely and accurate manner.
