Job summary

Prestige Care Center of Fairfield is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team on a per diem basis. The OT will evaluate patients, develop plans, and provide ADL/IADL training to maximize independence in daily activities.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and unrestricted Occupational Therapist license in the applicable state.
• Current CPR certification as required by facility policy.
• Experience in skilled nursing, rehabilitation, long-term care, or geriatric care preferred but not required.
• Strong clinical, communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Evaluate patients' functional, physical, cognitive, and psychosocial abilities and identify individualized occupational therapy needs.
• Develop, implement, and modify individualized treatment plans based on patient goals and response to treatment.
• Provide skilled occupational therapy services, including ADL/IADL training and therapeutic activities.
• Address dressing, bathing, grooming, toileting, transfers, and safety to promote independence.
• Collaborate with other healthcare professionals to support comprehensive patient care.
• Maintain accurate documentation of evaluations, treatment, progress, and discharge planning.
