Job summary

Hope Creek Nursing and Rehabilitation Center is seeking a dedicated Occupational Therapist (OT) to join our per diem team. You will evaluate patients, develop and implement individualized therapy plans, and work with an interdisciplinary team to promote independence and functional recovery in a skilled nursing setting.

The role emphasizes patient-centered care, collaboration with physicians, therapists, and families, and documentation of progress.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Current state licensure as an Occupational Therapist (OT).
• Ability to pass a background check and comply with our facility's policies and procedures.

Responsibilities

• Evaluate patients' physical and cognitive abilities and develop treatment plans to address their specific needs
• Implement and modify occupational therapy programs to promote patient progress and achievement of goals
• Provide individual and group therapy sessions, including exercises, activities, and education
• Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans
• Educate patients and their families on home programming and maintenance to support continued progress
• Document patient progress and treatment plans in a timely and accurate manner
