Job summary

Hope Creek Nursing and Rehabilitation Center in East Moline, IL is seeking a compassionate Physical Therapist (PT) to join our interdisciplinary team and help residents regain strength, mobility, and independence.

You will assess patients, develop individualized rehab plans, implement evidence-based interventions, educate on body mechanics, and document progress while supporting quality improvement and patient-centered care.

Qualifications

• Must be a licensed Physical Therapist (PT).
• Must have a strong foundation in physical therapy principles, techniques, and best practices.
• Must be able to work in a fast-paced, dynamic environment with patient-centered care.
• Must be able to lift, push, and pull equipment and supplies as needed.

Responsibilities

• Conduct thorough initial and ongoing assessments to identify patient needs and develop individualized rehabilitation plans.
• Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities.
• Collaborate with our healthcare team to develop and update patient treatment plans, ensuring seamless transitions and optimal outcomes.
• Conduct patient education and training on proper body mechanics, safety, and self-care techniques.
• Maintain accurate and detailed records of patient progress and treatment plans.
• Participate in quality improvement initiatives to drive continuous improvement and excellence in patient care.

Education

Degree from an accredited Physical Therapy program
