Qualifications

• Ability to travel to patient care assignments - Home Care only.

Responsibilities

• Evaluates, plans, directs, and administers programs of Occupational Therapy and plans and conducts OT programs to restore function, prevent disability, and help patients reach their maximum level of independence.
• Establishes and implements appropriate occupational therapy plans of care to restore function and reach maximum performance.
• Provides supervision in accordance with regulatory requirements, as appropriate.
• Completes and submits all documentation based on policies/standards and regulatory requirements.
