Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations. You will visit patients in their homes, typically ~45 minutes, and use an iPad with a clinical workflow. No prescribing or altering plans is required.

You will travel locally within licensed states, perform virtual visits as needed, and provide diagnostic and preventive services as indicated, bridging gaps in care for members.

Qualifications

• Active, unrestricted license(s) in coverage area(s) required or preferred.
• Board certification required for NP/PA roles.
• Multi-state licenses preferred for broader coverage.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Travel locally within licensed states as needed.
• Conduct virtual visits when required.
• Provide Signify Health services including diagnostic & preventive offerings (e.g., spirometry, bone density screening).
