Job summary

Signify Health, part of CVS Health, seeks Per Diem Clinician (NP/PA) to perform in-home health evaluations in members' homes across licensed states. Visits typically last ~45 minutes, include review of medications and history, with optional diagnostic testing as ordered by plans.

Role requires active NP/PA licenses, board certification, and willingness to travel locally. Per diem basis with hourly pay and benefits for part-time hours.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
