Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations, engaging with people in their own homes and bridging gaps in care.

The visits last about 45 minutes and include a medication/history review, physical evaluation, and optional tests as directed by the health plan. You may travel locally within licensed states and conduct virtual visits when needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide In-Home Health Evaluations.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Use iPad and clinical workflow for evaluations.
• Provide Diagnostic & Preventive Services as ordered.
