Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide in-home health evaluations. Visits last about 45 minutes, including medication and medical history review, a physical exam, and optional diagnostic tests.

You’ll use an iPad and a streamlined workflow; prescribing or changing treatments is not required. Candidates must hold an active, unrestricted NP/PA license and board certification.

Qualifications

• Active, unrestricted license in applicable states.
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations to health plan members.
• Use iPad and clinical workflow; perform history and physical exam.
• Travel locally within licensed states; conduct virtual visits as needed.
• Deliver Diagnostic & Preventive Services as indicated.
