Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner to provide in-home health evaluations and bridge gaps in care. The visit lasts about 45 minutes and uses an iPad with a clinical workflow.

You’ll travel locally to member homes, conduct reviews of medications and medical history, perform physical evaluations, and offer diagnostic services as ordered by health plans.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.)
