Job summary

Signify Health, part of CVS Health, seeks a Per diem Clinician (NP or PA) for in-home health evaluations. You will engage with members in their homes, review medical history and medications, and perform physical assessments using a streamlined iPad workflow. Travel locally as needed and provide diagnostic services as appropriate.

The role is part-time with a typical weekly load of 12 hours. Licensure must be active and unrestricted; board certification is required, with multi-state licenses

• Active, unrestricted licenses in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Review medication history and current therapies during visits.
• Perform physical evaluations and any ordered diagnostics.
• Use an iPad-based clinical workflow; do not prescribe or alter treatment.
• Travel locally to member homes within licensed states.
