ComplexCare Solutions is seeking a Nurse Practitioner for per diem coverage, delivering home and telehealth assessments across Jefferson County, Nebraska. This role emphasizes evaluating health status, gaps in care, and care management opportunities to improve outcomes.

Providers work remotely with flexible scheduling to meet patient needs. The position requires ANCC or AANP NP certification, an active license, CPR, and adherence to CMS guidelines.

Qualifications

• Active unencumbered nursing license.
• ANCC or AANP board certification as a NP/CS in Family, Adult, Gerontology or Emergency Medicine.
• Maintains current CPR certification.
• Vaccination/testing requirements prior to hire: Hepatitis B, Flu, MMR, Tdap, Varicella, TB.

Responsibilities

• Conduct comprehensive member assessments in home or telehealth settings.
• Document encounter findings per CMS/DHHS and client requirements.
• Address gaps in care and coordinate with PCPs/health plans.
• Maintain HIPAA-compliant documentation and secure communications.
• Perform and document Point of Care testing; follow up as needed.
• Provide member education on chronic conditions and preventive care.
• Escalate urgent cases per policy and professional judgment.
• Be available for ~37 hours per month (25 slots).
