Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem work covering all zip codes in Cass County, Nebraska. The role focuses on comprehensive home and telehealth assessments to evaluate health status, gaps in care, risks, and management opportunities with an emphasis on improving outcomes.

Providers conduct detailed one-hour assessments, document findings, and coordinate with PCPs and health plans.

Qualifications

• CMS Regulations and NCQA HEDIS knowledge.
• ADA/ACC/AHA guidelines; ICD-10 and CPT-4 familiarity.
• Thorough review of assigned member population.
• Document active medical conditions per CMS/HIPAA.
• Support physician/patient relationship and communication.
• Provide patient education to improve chronic condition knowledge.
• Close gaps in care (disease-specific or preventive).
• Identify urgent situations and escalate per policy.
• Perform and document Point of Care testing.
• Maintain compliance with company policies.
• HIPAA confidentiality adherence.
• Monthly availability: minimum 25 slots (~37 hours).
• ANCC or AANP board certification as NP.
• Active unencumbered nursing license.
• Current CPR certification.
• Vaccination/testing compliance before hire ( Hep B, Flu, MMR, Tdap, Varicella, TB).
• Home health experience is a plus.
• Able to communicate with elderly and chronically ill patients.
• Understanding of Medicare/Medicaid and health plan benefits.
• Ability to multitask.

Responsibilities
