Job summary

Optum Home & Community Care seeks experienced Advanced Practice Clinicians for a per diem role in Oregon. You may choose hours and must be available at least 24 hours per month, joining a team delivering care in nursing homes, senior housing and assisted living settings.

NP or PA credentials are required with active state licensure and DEA where applicable. Compensation ranges with base pay and bonus, plus a robust benefits package and 401k options.

Qualifications

• Active ANCC or AANP certification for NP role or NCCPA for PA role.
• Current, unrestricted licensure in the practicing state.
• DEA licensure or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments and document findings.
• Support sub-acute rehabilitation episodes and care planning.
• Collaborate with interdisciplinary care teams and caregivers.
• Provide counseling and education to patients and families.
• Maintain accurate ICD-10 coding and documentation.
