Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care program to deliver integrated clinical care for high-risk members in long-term care and home settings. The Advanced Practice Clinician role focuses on comprehensive assessments, care planning and coordination within an interdisciplinary team.

Per diem position with no on-call, no weekends, and no holidays. Compensation ranges hourly from $53 to $79, with benefits and growth opportunities available based

Qualifications

• Required: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state
• Active and unrestricted driver's license; access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and perform job duties

Responsibilities

• Conduct comprehensive assessments including medical history review, medication reconciliation, vital signs, and physical exam
• Provide support during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation including ICD-10
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide counseling and education to patients and caregivers
