Job summary

UnitedHealth Group, through Optum Home & Community Care in Oregon, is hiring for a per diem Advanced Practice Clinician to serve in senior living and home care settings. You will assess patients, coordinate care, and support the interdisciplinary team to ensure quality outcomes.

The role offers flexible hours, requires NP or PA certification with licensure in your state, and involves documenting care, collaborating with care teams, and engaging patients and families in planning.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state without disciplinary actions
• Active and unrestricted driver's license with access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself to perform job duties

Responsibilities

• Conduct comprehensive assessments, including medical history review, medication reconciliation, vital signs, and physical exam
• Support members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education
