Job summary

UnitedHealth Group is seeking a per diem Advanced Practice Clinician (NP/PA) to provide care in senior living and related settings within Oregon. You may choose your hours and must be available at least 24 hours per month, with opportunities to work in nursing homes, senior housing, and assisted living environments.

The role emphasizes comprehensive assessments, care coordination, and advanced illness planning, with a team-focused approach to improve patient outcomes and access to care.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NP)
• Current NCCPA certification and state licensure (PA)
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date)
• Active and unrestricted driver’s license with reliable transportation
• Current, active DEA licensure/prescriptive authority (or ability to obtain post-hire)
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself to perform job duties

Responsibilities

• Conduct comprehensive assessments including medical history, medication reconciliation, vital signs, and physical exams
• Support members during short-term sub-acute rehabilitation episodes
• Ensure accurate documentation including ICD-10 conditions
• Collaborate with the interdisciplinary care team
• Engage in advanced illness and end-of-life care planning conversations
• Provide counseling and education to patients and caregivers
