Job summary

Optum Home & Community Care (UnitedHealth Group) is seeking a per diem Nurse Practitioner to support the Senior Community Care program, focusing on high-risk members in home and facility settings. Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontology, or AG-PC.
• Active, unrestricted NP license without disciplinary actions.
• Active DEA licensure/prescriptive authority (or ability to obtain post-hire).
• Ability to establish a collaborative practice if required by state regulations.

Responsibilities

• Conduct comprehensive assessments and medication reviews.
• Perform point-of-care testing and advanced care planning discussions.
• Collaborate with an interdisciplinary care team to support seniors’ rehabilitation and ongoing needs.
• Provide care across home, nursing home, assisted living, and senior housing environments.
